Bridget Clark PT, MSPT, DPT has been practicing physical therapy and treating patients with vertigo for eight years.  After completing her doctorate studies at Arcadia University (ranked nationally in the top ten for PT programs), she began to see a pattern in vestibular treatment. Identifying the nystagmus (rapid eye movements) is essential to determining what canal is affected and then providing the correct treatment.  However, in most cases, the nystagmus happens so quickly that it is difficult to identify the direction of the fast phase, especially as a newcomer to the treatment.  When you think about it, it makes perfect sense. When a clinician goes to a course to learn a new technique, typically, they return home and try it on every willing family member.  It's easy to practice a new soft tissue technique or joint manipulation since just about every person has some degree of dysfunction however it's not as easy or available to practice identifying positional vertigo. Even with patient demonstrations, the nystagmus occurred so quickly that the students weren't able to observe what was happening.

Science behind the treatment

Benign Paroxysmal Positional Vertigo (BPPV) is a condition in the inner ear caused by the misplacement of calcium carbonate debris (otoconia) in the semicircular canals. Common symptoms of BPPV include short lasting vertigo that occurs when the person looks up, bend forward or rolls over in bed. The vertigo usually has a slight delay at onset and lasts less than 1 minute. A patient with BPPV may also experience nausea and imbalance. 

BPPV can occur spontaneously, after a head injury or after an inner ear infection such as labrynthitis.

There is no other treatment that you can provide that can come close to the success rate of canal repositioning.

There are two types of BPPV. The first and more common type, canalithiasis, is due to free floating otoconia in the semicircular canals. In this case the vertigo will only last a few seconds. The second type is called cupulolithiasis and occurs when the otoconia is adhered to the cupula within the semicircular canal. Patients with this condition will have longer lasting vertigo. Both conditions can be treated successfully without any additional equipment, medication or staff. 

The vertigo experienced in both conditions is associated to the nystagmus (a rapid eye movement defined by the fast phase of movement) produced in the eye as a result of the particle moving in the semicircular canal. The direction of the nystagmus lets the clinician know which canal is affected. Once the canal is identified, the patient is treated with a simple repositioning maneuver to remove the debris from the canal. These maneuvers have been clinically proven to be up to 95% effective in only 1-2 visits (Epley 92, 96, Herdman et al 93, Weider et al 94, Welling et al 94, Fung et al 96).
